All-on-4 or All-on-6 — which is right for you? Dr Dušan Miljuš explains

Posted on 10/20/2026

“All-on-4 or All-on-6?” is one of the questions we hear most when several teeth are missing or someone wants to finally say goodbye to a denture. The answer isn't “one is better” — it depends on the bone you have. Dr Dušan Miljuš, oral surgery specialist, explains.

Dr Dušan Miljuš: All-on-4 or All-on-6? (40 seconds, in Serbian with German subtitles)

The short answer

“Whenever we can place five, six or more implants, we go for more implants — because it lasts much longer and is much better for the patient.”

Both give you fixed teeth on implants — teeth that don't come out. The difference is how many implants they rest on, and how much bone is left.

All-on-6: when there is enough bone

“With All-on-6 or All-on-8 we have much more tissue, so the teeth are shorter, the aesthetics are better, the function is better — everything is much easier.”

All-on-4: the “last chance” for fixed teeth

“These patients often have a lot of bone loss, so we barely have four positions for implants. We call it the last chance to get fixed teeth.”

The longer a denture is worn, the more bone is lost — and the fewer options remain. If you're thinking about fixed teeth, it's better to check your bone today than in five years.

How safe is it?

“These procedures are very predictable — over 98% successful, fewer than 1% complications.”
