All-on-4 vs All-on-6: Difference, Price per Jaw and Which Is Better

Posted on 11/4/2026

Short answer

Both give you fixed teeth on implants for a whole jaw. The difference is the number of implants: All-on-6 has six supports and is the better choice when there's enough bone, while All-on-4 is the answer when there's little bone. Implant survival after 5 years is almost the same for both, around 99%. With us, All-on-4 costs €4,000 and All-on-6 €5,000 per jaw. Your dentist decides how many implants you need from a CBCT scan.

All-on-4 vs All-on-6: number of implants, price per jaw, when each is the right choice, warranty

What's the difference between All-on-4 and All-on-6?

With All-on-4, four implants hold the whole bridge for one jaw; with All-on-6, six do. The two back implants in All-on-4 are placed at an angle to use the bone that's left and often avoid a bone graft.

Which is better: four or six implants?

When the bone allows it, more implants are better. Six supports spread the chewing force better, and the teeth on the bridge can be shorter and look more natural.

Dr Dušan Miljuš, an oral surgery and implantology specialist in Belgrade, puts it simply (translated from Serbian): "Whenever we can place five, six or more implants, we of course go for more implants, because it lasts much longer and is much better for the patient." With All-on-6 there's more tissue to work with, so, as he explains, "the teeth are shorter, the aesthetics are better, the function is better".

Dr Miljuš describes All-on-4 as the "last chance" for fixed teeth: usually patients who have worn a removable denture for years and lost a lot of bone, leaving barely four positions for implants.

Is All-on-4 less successful than All-on-6?

Not meaningfully. In a large study of 943 patients and almost 6,000 implants, 98.8% of implants survived after 5 years with All-on-4 and 98.7% with All-on-6.[1]

Long-term All-on-4 results are good too: a systematic review found 99% of implants surviving after three years[2], and a 10- to 18-year follow-up in the lower jaw found 98.8% of bridges and 93% of implants still working.[3] So the difference between four and six implants isn't whether it "works", but how much bone you have and how long the work lasts without complications.

How much bone do you need for All-on-6?

Enough to place six implants without a major bone graft. Only a CBCT (3D) scan shows this, because it measures the height and width of the bone across the whole jaw.

That's why sending your scan before you travel matters most. If you don't have one, it can be done at the clinic for an extra fee. A bone graft or sinus lift, if needed, is charged separately and listed in your plan.

What decides between All-on-4 and All-on-6: enough bone leads to All-on-6, little bone to All-on-4

Why not wait too long with a denture?

Because a removable denture slowly wears the bone away, leaving less and less room for implants. "Everything is much easier if patients come in time, if they don't wear removable dentures for too long, which melt away the bone," says Dr Miljuš. People who come earlier more often get to choose between All-on-4 and All-on-6.

Frequently asked questions

How much does All-on-6 cost in Serbia?

With us, €5,000 per jaw, including implants, abutments, extractions, temporary and final bridge. All-on-4 costs €4,000.

Can I choose All-on-6 myself?

You can say what you'd prefer, but the number of implants depends on your bone. The dentist will tell you from your scan whether All-on-6 is possible without a major graft.

Does All-on-4 last less long?

Studies show similar implant survival after 5 years. Dentists still place more implants whenever the bone allows, because the work is then more stable in the long run.

Does implant placement hurt?

It's done under local anaesthetic. Placing one implant takes a few minutes. Sedation is available for an extra fee.

When do I get my teeth?

The fixed temporary bridge usually the next day, the final bridge after about 3 months on your second trip.
